Loss Of Original, Duplicate And Triplicate Copies Of Accountable Form No. 56

Notice is hereby given to the public of the loss of Box No. 21 of Accountable Form No. 56 Real Property Tax with Serial Number 257001-258000 last January 20,2022 of Municipality of Polomolok. Incident hapen when we made the final delivery of forms during unloading of boxes.
This Notice is being issued in order that all concerned may guard against the fraudulent use of the same. When found or discovered to be in possession of unauthorized person, it is requested that the matter be immediately reported to the Municipality of Polomolok Treasurers office. Any transaction involving the said Accountable form shall not be honoured and considered illegal.
